Apply for Assistance

The Center for Independence has been serving people with disabilities on the Western Slope of Colorado since 1982. Our services include peer counseling, personal systems advocacy, information and referral, support groups, transitional support, classes and education. Our services are available to people with all types of disabilities. Below you have the option to choose between two applications for services, please review each and apply for those that are most relevant to you.

General services include housing, transition, youth, peer support, advocacy, deaf/hard of hearing, independent living skills, blind/low vision and vocational assistance. You should hear back from a staff member within 10 days of applying for services. 

Currently we are accepting new SSI/SSDI benefits cases online. We are operating on a waitlist and encourage you to fill out the form here (by clicking the blue link) to be placed on it. A staff member will call you as soon as possible with more information. 

If you prefer a physical application we have them available at your local office. For the Grand Junction office we will be able to assist you from the walk-up window in the parking lot.
